Move and Delivery Application Form

Requirements for Moves or Deliveries

1. Separate requirements exist depending on whether the owner is engaged in a full household

move or is accepting a delivery of an appliance or one or two pieces furniture and are described

in the requirements below.

2. Regardless of the type of move (full household or delivery) owners are responsible to advise the

designated property management company and the Board that a move is eminent. In the event

of a full household move, the notification must be given writing no less than 10 business days in

advance of the move. Notice of a delivery must be provided at least 48 hours in advance. This

will allow property management to determine if there is a conflict with scheduled contractor work

or a previously approved move. No move shall be made without proper notification and

confirmation by the designated property management company. Failure to make proper

notification will result in a fine.

3. The Move Application found on the FSIV website must be used to request a move. A date and

time of the move and/or delivery must be provided. If a mover is used for a full household move

the name of the mover and a certificate of insurance covering potential damages to the common

areas must be provided. Franklin Square Condominium IV shall be named as an additional

insured.

4. Owners are responsible for hanging the pads in the freight elevator (north elevator with higher

ceiling) for a household move or a delivery of unboxed or oversized items. The pads are provided

by the Association and can be found in the 2nd floor trash room along with hangers and a ladder.

In addition, a rug, also provided in the 2nd floor trash room, must be put on the floor in the freightMove and Elevator Padding Resolution continued

elevator for moves or deliveries. Only the freight elevator (higher ceilng)can be used for a move

or delivery. Deliveries of boxed items do not require padding of the elevator. An owner can hire

someone to put up pads and the rug and remove them as well; if so, this must be noted on the

application.

5. Any move must be supervised by the owner of the unit who must be on-site for the move. If the

owner is not able to be on-site then a responsible individual must be identified on the application

who will be on-site for the entirety of the move. Owners are responsible for insurance coverage

and reimbursement to this individual if required.

6. A day is defined as a day beginning no earlier than 9 AM and ending no later than 5 PM. If the

household move should require multiple days for completion (more than 6 hours), the property

management company and the Board must be notified. Generally, moves and/or deliveries must

take place on a weekday. Exceptions to this requirement must be approved by the Board in

advance.

7. Only one move will be allowed on any one day; the first completed and approved application will

be the allowed move for that day.

8. In no instance during the move shall:

- any elevator other than the freight elevator be utilized (the north elevator);

- the front door of the building be propped or left open;

- items be left on the sidewalk in front of the building; or

- items left in the first floor lobby and/or hallway on any floor.

FSIV is a non-smoking building and no smoking will be tolerated in the building or on the sidewalks

in front of the building. The owner (or the designee) will instruct any moving company/staff to

comply with these rules and to be courteous of the building.

9. All elevator pads and the rug must be removed the same day as the delivery or move. Pads and

the rugs must be returned to the 2nd floor trash room in the same condition as received.

10. In no instance will moves be authorized for more than one unit in a single day.

11. All owners are solely responsible for the move and any damages to any part of the building that

may occur during the move.

12. Any violations of this resolution are subject to a fine determined by the Board and added to the

owner’s account.
